In this article, we distill the following 5 evidence-based arguments that commonly motivate patients to select radical prostatectomy as initial treatment of 'high-risk' prostate cancer. The first is that surgery alone provides excellent long-term cancer control and disease-related survival. Second, radical prostatectomy provides definitive local and regional disease staging, serving as the backbone for multimodality treatment if needed. Third, the sequencing of surgery before radiation is viewed as preferable beca by preserving options for effective local salvage. Fourth, initial surgery successfully defers or avoids the adverse effects of long-term androgen deprivation therapy in a substantial proportion of patients. Lastly, fifth, prostatectomy offers manageable impacts on urinary and sexual function, minimizing risk of gastrointestinal toxicity or secondary malignancy, and averts local symptomatic progression in most.

In this real-world study of patients initially presenting with non-metastatic disease who subsequently received PARP inhibitors, transcriptome analysis from the primary tumor revealed the presence of adverse molecular features when disease was still localized. Patients with higher Decipher score, p53 mutation signature and PTEN inactivation tumors at initial testing, could be considered for additional confirmatory genomic or genetic testing and potentially novel trials of PARPi.

This study investigates tumor-associated macrophages (TAMs) in renal cell carcinoma (RCC) and their role in resistance to immune checkpoint inhibitors (ICIs) through single-cell analysis. The research identifies specific TAM populations, particularly the LILRB/SIGLEC10-TAM subcluster, that are associated with ICI treatment resistance, providing insights for potential therapeutic targets in RCC.

KYNU is a robust prognostic biomarker and potential immunometabolic target in LUAD, especially in STK11 and KEAP1 co-mutated tumors. Its cancer cell-intrinsic expression and immunosuppressive metabolic phenotype offer translational potential, though species-specific expression patterns pose challenges for preclinical modeling.

For patients with advanced prostate cancer (PC) treated with androgen deprivation therapy (ADT) plus an androgen receptor pathway inhibitor (ARPI), the decline in prostate-specific antigen (PSA) is a potential biomarker for treatment response. A novel approach extends Dropout to the frequency domain, allowing stochastic attenuation of signal frequencies during inference. This creates diverse global textural variations in feature maps while preserving structural integrity - a factor we hypothesize and empirically show is contributing to accurately estimating uncertainties in semantic segmentation.

Our proposed SRE U-Net not only significantly surpasses standard U-Net in handling rotated images, but also outperforms existing equivariant learning methods and does so with a reduced number of trainable parameters and smaller memory cost.

The role of treatment of the primary tumor, by either radiation therapy or cytoreductive prostatectomy, in low-volume disease continues to be explored. As novel therapies, targeted agents, and immunotherapies undergo investigation, optimizing treatment selection based on disease burden, molecular characteristics, and patient factors will be essential.
