  • 00:00Hello, my name is Mark Di Capua,
  • 00:03one of the technology trainers at Yale,
  • 00:05New Haven Health and Yale Medicine.
  • 00:07This video guide will show you how
  • 00:09to download, configure, and use my
  • 00:11chart for the purpose of video visits,
  • 00:13let's go ahead and get started.
  • 00:16Before we begin, you will need to
  • 00:18have a my chart account with Yale,
  • 00:20New Haven Health, and Yale Medicine.
  • 00:22If you do not have a my chart account,
  • 00:26please contact your provider
  • 00:28or visit my chart. YNH s.org.
  • 00:30Please note that it is recommended
  • 00:32that you download the my chart
  • 00:35app in order for video visits to
  • 00:38work properly on your iOS device.
  • 00:40Please do not use my chart to send any
  • 00:43messages requiring urgent medical attention.
  • 00:46For urgent needs, please contact your
  • 00:50provider's office or emergency services.
  • 00:53To download my chart,
  • 00:54open the App Store on your iOS
  • 00:57device by tapping on the icon.
  • 01:00Tap the search window and type my chart.
  • 01:03My chart will return as a search result,
  • 01:06tap my chart to continue.
  • 01:09To ensure you have the correct
  • 01:11my chart app for Yale,
  • 01:13New Haven Health and Yale Medicine,
  • 01:15look for the icon showing a red
  • 01:17folder with a heart and the title.
  • 01:19My chart.
  • 01:20Your secure health connection.
  • 01:21Tap the get icon to download my chart.
  • 01:24You may need your Apple ID and password to
  • 01:27download based upon your device is set up.
  • 01:30Please note if you have
  • 01:32previously downloaded my chart,
  • 01:33a cloud with the Down Arrow will appear.
  • 01:36Click the cloud icon to re download my chart.
  • 01:41Now that my chart has downloaded
  • 01:43onto your device,
  • 01:44tap the my chart icon.
  • 01:48Tap except to accept the
  • 01:51end user license agreement.
  • 01:53My chart will ask you if you wish to
  • 01:56allow my chart to access your location.
  • 01:59Choose allow while using app.
  • 02:03The my chart app will recognize your
  • 02:05location and present you with a list of
  • 02:09participating healthcare organizations.
  • 02:10An alphabetical order to find Yale,
  • 02:12New Haven Health and Yale Medicine.
  • 02:15Scroll down.
  • 02:17Tap Yale New Haven Health Yale University.
  • 02:20My chart to appropriately configure my chart.
  • 02:24The my chart app will alert you that you
  • 02:26can switch organizations in the future
  • 02:29by using the switch organizations,
  • 02:31but and this may be useful if
  • 02:33you see a provider in a different
  • 02:35organization that uses my chart tap OK.
  • 02:38Got it to proceed.
  • 02:41Log into my chart by entering
  • 02:43your username and password.
  • 02:44Then tap the login button.
  • 02:47If you forgot your username or password,
  • 02:49tap the forgot password or username text.
  • 02:53If you do not have a my chart account,
  • 02:56either contact your provider
  • 02:58or tap sign up now.
  • 03:01Once you have logged into my chart,
  • 03:04you will be asked if you want my
  • 03:06chart to send you notifications.
  • 03:08Select the option that best suits your needs.
  • 03:12Next, my chart will ask you if you wish
  • 03:14to log in faster using a passcode.
  • 03:17Select the option that best works for you.
  • 03:22Lastly, my chart will notify
  • 03:23you of your notification status
  • 03:25and alert you to any messages.
  • 03:27Appointments in other reminders
  • 03:29you may have simply tap away from
  • 03:32the popup window to continue.
  • 03:35You will see that my chart
  • 03:36has a number of options,
  • 03:38including test results messages,
  • 03:40an billing since we are focused on getting
  • 03:42you connected to a video visit appointment,
  • 03:45tap on appointments.
  • 03:48My chart will show your scheduled video
  • 03:50visit along with any additional visits
  • 03:52that have been scheduled in the past.
  • 03:55Please note if you do not
  • 03:57have a video visit scheduled,
  • 03:59please call your provider's office and
  • 04:01schedule a video visit as you normally would.
  • 04:04Irregular visit.
  • 04:05If you have a video visit scheduled,
  • 04:08tap on the visit.
  • 04:11Complete the check in process
  • 04:13by tapping E check in.
  • 04:15You will be required to sign
  • 04:17any outstanding documents,
  • 04:18including the Tele health consent.
  • 04:21After you have finished the check in process,
  • 04:23you will receive a message that
  • 04:25your E check in is complete.
  • 04:28Tap the appointment.
  • 04:29Before you begin your visit,
  • 04:31considering the following.
  • 04:33Make sure you are in an area with
  • 04:36a strong network or Wi-Fi signal.
  • 04:39Turn on your microphone and speakers.
  • 04:41Turn off your Bluetooth unless it
  • 04:43is being used for medical device.
  • 04:45Close any open additional apps.
  • 04:49At the bottom of the screen you
  • 04:51will see the begin visit bar.
  • 04:53When you're ready to start your video visit,
  • 04:56tap begin, visit.
  • 04:59When you tap begin visit,
  • 05:00video connect will launch in
  • 05:03your devices web browser.
  • 05:04To join your visit, tap join via the browser.
  • 05:09Tap allow to allow video connect to
  • 05:12access your device is camera and microphone.
  • 05:15Note if you do not tap allow your video
  • 05:18visit will not function properly.
  • 05:21Type your name in the enter your name field.
  • 05:25Tap the box next to agree to the video
  • 05:29user terms and conditions and privacy policy.
  • 05:33Now tap join.
  • 05:34You are now joined to the video visit
  • 05:37when the provider joins the visit,
  • 05:40you will see yourself in the lower
  • 05:42right hand corner of the screen.
  • 05:44Get ready to smile.
  • 05:46Please be patient and allow some
  • 05:48time for your provider to join
  • 05:50the video visit session.
  • 05:52Once your provider joins,
  • 05:54please speak directly into the
  • 05:56microphone and be aware of the
  • 05:58direction the camera is facing.
  • 06:00If you find it hard to hear your provider,
  • 06:03try turning up the volume on your device.
  • 06:07You may disable your video or
  • 06:09mute your microphone by tapping
  • 06:11on the controls at the bottom.
  • 06:13Once the video visit has concluded,
  • 06:16you may tap the Red Disconnect Button.
  • 06:19Once you have disconnected,
  • 06:21you may close your browser window
  • 06:23and the my chart app.
